My health for life
A free, six-month program helping participants achieve their health goals.

Based in the community and delivered by local healthcare practitioners, My Health for Life identifies those at high risk of developing chronic conditions and offers them a tailored, but flexible, healthy lifestyle program.
My health for life works in partnership with GPs, providing patients an opportunity to participate in an evidence-based lifestyle modification program in their local community. Facilitators are trained health professionals, including dietitians and exercise physiologists, who have a keen interest in preventive health.
Although GP referral is recommended, referrals can also be made by a practice nurse, allied health professional or the patient themselves.

My Health for Life group sessions
GPs and health professionals can refer patients to group sessions that participants can choose to complete via face-to-face, video conferencing or telephone coaching.
The program works in partnership with general practice and is a practical extension of the advice given by GPs and nurses to their patients.
