About us

Our vision is a community where good health is available for everyone. We work with others to create and deliver the best possible health and community care system for the people of our region.

About us header

We are one of 31 Primary Health Networks across Australia.

We support clinicians and communities in Brisbane’s northern suburbs, Moreton Bay Regional Council, parts of Somerset Regional Council and support to the people of Norfolk Island.

Our region covers approximately 4100 km2 of urban, regional and rural areas, with a population of over one million.

Our key objectives are to:

  • increase the efficiency and effectiveness of medical services for patients—particularly those at risk of poor health outcomes
  • improve coordination of care to ensure patients receive the right care, in the right place, at the right time.

Year in Review

Each financial year Brisbane North PHN produces an annual report, or a ‘Year in Review’ for our board members and stakeholders to showcase our work over the previous 12 months.
