HealthPathways
The HealthPathways program is a joint project between Brisbane North PHN and Metro North Hospital and Health Service.
HealthPathways is designed to provide our region's clinicians with up-to-date evidence-based patient assessment and management, referral and service information in an easy-to-access web and mobile-friendly format.
The HealthPathways website is integrated with GP Smart Referrals, ensuring faster, streamlined management of referrals within Queensland public hospitals.
The site continues to expand with the addition of new pathways supporting primary care and responding to emergent needs, such as the COVID-19 suite of pathways.
