General Practice Liaison Officer program

The GPLOs are a team of experienced GPs and an Optometrist with general/private practice backgrounds.

GPL Os 2024

The General Practice Liaison Officer (GPLO) program is a partnership between Metro North Hospital and Health Service and Brisbane North PHN.

The GPLOs are an experienced team of GPs and an Optometrist who bring their unique insights to improving system integration between hospital and primary care services.

GPLO activities include:

  • facilitation of multi-disciplinary GP and health provider education events
  • practice visits to support the use of integrated systems including Health Provider Portal, GP Smart Referrals and HealthPathways and discuss new services and patient pathways
  • working to improve clinical handover between hospital to primary care and enhancing continuity of care
  • providing primary care advice to key working groups and contributing to health service planning and the development of pathways of care and state-wide referral guidelines
  • designing and implementing new models of care including shared care models
  • providing GPs with regular communications through the PHN’s weekly newsletter, GP Link.
