Achieving excellence: Team Care Coordination's journey to accreditation
10 February 2025Team Care Coordination (TCC) is an established and trusted partner in the North Brisbane and Moreton Bay region, supporting residents with chronic and complex conditions.
We are proud to congratulate the TCC team who have successfully met the National Safety and Quality Primary and Community Healthcare Standards to achieve accreditation.
Accreditation is a formal program wherein independent reviewers meticulously assess adherence to and implementation of these standards. This prestigious recognition ensures that our healthcare service delivery meets the highest safety and quality standards.
The assessor was particularly impressed with TCC's approach to continuous quality improvement (CQI) and how well CQI was embedded into the program, as well as the work the team is doing in the consumer engagement space through the Consumer Advisory Body.
Our TCC team's commitment to helping adults across our region with complex and chronic health conditions to live well at home is inspiring. This milestone highlights their dedication to delivering first class care and support to consumers.
