An impactful 2023 for Team Care Coordination

To wrap up an impactful year, Team Care Coordination are sharing a snapshot of our 2022-2023 Annual Report which encapsulates how immensely valuable quality care coordination can be for ensuring better health outcomes for clients of the program.

After more than 25 years of service, Team Care Coordination (TCC) remains committed to improving clients’ experience managing their chronic health conditions, and the impact this can have on their health and ability to live an independent life.

TCC are proud to share these achievements with you – notably, the increase in the number of referrals to the service in 2023, up 20 per cent on the previous year. This is due in part to the support and partnership of the Health Alliance’s Care Collective project, and ongoing engagement and connection building with our Hospital and Health Service (HHS), local GPs and practice nurses and direct client engagement to promote self-referrals to the program.
