Brisbane North staff represent at the 2023 PHN Commissioning Innovation Showcase
03 October 2023PHN Commissioning Innovation Showcase event focused on accelerating outcomes and equity through commissioning.
This coveted annual event included representation from PHNs Australia-wide coming together to showcase innovation across themes in system reform and leadership, commissioning with partners, valued based commissioning and streamlining PHN commissioning.
Sharon Sweeney and Anu Manoharan from our Healthy Ageing team presented on the lessons learned from the Commonwealth Home Support Program Consortium outlining the challenge, approach, impact and learnings from this commissioning model. Health Alliance Program Manager – Complex Care Models, Alison Berigan delivered an engaging presentation on the Care Collective, a program designed to support people living with complex chronic health conditions to navigate and integrate into the healthcare system within the Caboolture region.
