Care Collective bringing service providers together to empower clients: Essential Health Summit wrap up
22 August 2024Last month the Health Alliance’s Care Collective travelled to Adelaide to present at the Essential Health Summit for health and wellbeing professionals.
Jointly hosted by the Australian Primary Health Care Nurses Association (APNA) and the Australasian Society of Lifestyle Medicine (ASLM), this year’s inaugural event explored the latest developments in health and lifestyle medicine from neuroscience, physiotherapy and nutrition to general practice, primary care and health science.
The Care Collective program provides practice nurses with the opportunity to upskill into Complex Care Coordination (CCC) roles, supporting them to work to their full scope of practice whilst keeping people with complex chronic disease in the community and out of hospital emergency departments.
The Summit was an ideal opportunity to share with colleagues the Collective’s success in both the Caboolture and Redcliffe regions, achieved through coordinating care pathways for these patients that allow them better access to general practice and multidisciplinary care teams.
