Care Collective celebrates primary healthcare nurses
05 February 2025This Wednesday 5 February, the Health Alliance's Care Collective team will celebrate primary healthcare nurses working across the Caboolture and Redcliffe regions of our Brisbane North PHN catchment.
This celebration marks Primary Healthcare Nurses Day which recognises the contributions that health professionals make to treating the whole person in primary care, highlighting the unique blend of science, expertise and compassion they bring to their roles.
This contribution has been demonstrated firsthand during these past two years of the Care Collective. To date, the primary healthcare nurses involved in the program have enthusiastically embraced their roles as Complex Care Coordinators (CCC) in the participating GP practices. The CCC role provides an opportunity to show the positive impact of treating the whole person by offering a holistic assessment and coordination of support, services, and education that supports clients with chronic health needs.
The Care Collective has supported the induction and training of 25 primary healthcare nurses to deliver the CCC role in 18 practices across our region. This team of skilled health professionals has supported over 700 patients and their families, offering a client-centred approach to the management of each of these individual's chronic health conditions.
The result of this work is almost 5000 interactions with or on behalf of enrolled Care Collective patients, including referrals to other health or social programs in the area.
Feedback from these patients that speaks to the impact of these interactions on their lives is incredibly positive. We're proud to report that:
100 per cent of patient feedback reported a positive outcome arising from these connections.
The Care Collective and broader Health Alliance and Brisbane North PHN teams appreciate the work of primary healthcare nurses in these Complex Care Coordination roles, and would like to thank each and every one of them for their valuable contribution to better patient outcomes and holistic, person-centred care.
