Care Collective hosts Complex Care Coordinator Workshop
30 November 2023The Brisbane North Health Alliance Care Collective program continues to go from strength to strength as the program expands in the Caboolture region with eight general practices now participating.
In October, a follow up Complex Care Coordination workshop (the second in a series) was held in North Lakes with a focus on strengthening the connections between the Care Collective Complex Care Coordinators working in general practice and their health professional counterparts in the Metro North region.
The Collective relies on the shared experience of Nurse Navigators, Discharge Planning Nurses and Clinical Curse Consultants, Clinical Leads from the Complex and Chronic Disease Team (CCDT), the Rapid Access Community Care (RACC) team, and Team Care Coordination Service (TCC) and Engagement Officers from Brisbane North PHN, all who were in attendance on the day and pictured below.
Presentations highlighted referral pathways, eligibility criteria and models of care to assist and support the Complex Care Coordinators in their new roles in general practice. Participants worked together in groups to put their collective knowledge and experience together to work through client case studies and coordinate care for Care Collective clients.
Whilst many in the room had been practice nursing for many years, they reported the opportunity to build more robust connections with clients, and spend more time with them actively coordinating their care in the region through the Care Collective program was appealing.
