Care finder helping older Australians to navigate aged care

Brisbane North PHN has established a new program to help highly vulnerable people to navigate and access Australia’s Aged Care system.

Six local organisations have been commissioned to manage a workforce of care finders across the north Brisbane and Moreton Bay region and will provide intensive, face-to-face support to help people navigate and access aged care services.

The care finder program is specifically designed to assist older Australians with complex needs who have no support or are isolated and who find it difficult or are hesitant to access aged care services.

Being rolled out nationally, in response to recommendations from the Royal Commission into Aged Care Quality and Safety, the workforce of care finders aims to improve outcomes for older Australians by ensuring they have a better understanding of aged care services, providing support through the assessment process, connecting people to services and following up to ensure services continue to meet their identified needs.

Communify Queensland (servicing Brisbane), OzCare, Footprints Community, Communify Queensland, Queensland Council for LGBTI Health, Micah Projects (Lotus Place) and World Wellness Group are providing care finder services in the north Brisbane and Moreton Bay region.
