Head to Health has a new home

Same service, new provider.

From Monday 16 December 2024 Head to Health service navigation will no longer be provided by the Brisbane North PHN, and will instead be delivered by Neami National.

Brisbane North PHN joined the Head to Health network in 2022 and has had a busy two years with 149 calls to Head to Health made in October 2024 alone and an 86 per cent overall experience rating.

Who has the Brisbane North PHN Head to Health team supported?

  • 21.1 per cent of all contacts came from justice or forensic pathways (Queensland Police referrals)
  • 18.5 per cent of all contacts were self-referrals
  • 14.4 per cent of all contacts came from GPs
  • 69.9 per cent of consumers were between aged18 and 64 years old
  • 9.1 per cent identified as LGBTQIASB+.

The new Head to Health service navigation team at Neami National will provide the same service under the same name, and same number. When you call the national Head to Health network number 1800 595 212 (free call) you will be asked to enter your postcode and will then be transferred to a Head to Health service navigator at Neami National.

Just like the current service, Head to Health service navigation as provided by Neami National can still be accessed by anyone requiring support including individuals and their families or carers, GPs, service providers, and other health professionals. The Head to Health team will ask you a range of questions to connect you, or the person you are supporting, with the most appropriate service.

The Head to Health phone line will remain operational between 8.30 am to 5.00 pm, Monday to Friday (excluding public holidays). If you call Head to Health outside of operational hours, you will have the option to leave a message and will receive a call back the next business day.
