Innovation: Stakeholders showcase Care Collective's impact

Since 2022, the innovative Care Collective program has been strengthening general practice and changing lives.

To celebrate the many achievements of the Care Collective since its pilot in Caboolture and expansion into Redcliffe, the Health Alliance convened the Care Collective Showcase in March, bringing together stakeholders including hospital clinicians, GPs, primary care nurses and community services, all promoting the program’s impact.

At the core of Care Collective’s model of care is building general practice capacity to support clients with complex chronic health conditions. This approach has proven not only beneficial to the more than 800 clients supported over the last three years but has also demonstrated significant improvement to the local health system, having considerably reduced unnecessary emergency department presentations.

At the showcase, participants including the program’s Complex Care Coordinators and clinicians presented their achievements and shared learnings and opportunities for continued improvement.

GP Dr James Martin and Complex Care Coordinator Lorraine Evans from Pebble Beach Medical Centre, Sandstone Point presented on establishing their case conferencing model which has emerged in their practice through the Care Collective program. Dr Martin and Loraine outlined the steps they took to ensure they regularly met to discuss Care Collective clients' complex health conditions and devise plans to ensure they remained well and living in their own homes for as long as possible.

The showcase successfully reinforced the Care Collective's impact across the Caboolture and Redcliffe regions, building on the highlights of the program's evaluation which demonstrated:

  • 450 per cent return on investment
  • $429,000 saving back into the health system per month
  • 63 per cent reduction in ED presentations for Care Collective clients.

The Health Alliance welcomes the opportunity to continue to advocate for the Care Collective's continuation in 2025-26 and beyond, enabling continued care for current clients while supporting ongoing health system improvement efforts.
