Integrated Assessment Tool Live Trial
28 August 2023Partners 4 Health Regional Assessment Service took part in the Department of Health’s new Integrated Assessment Tool (IAT) Live Trail, which ran from April to late July this year.
Our Care Coordination team was excited to work with the Department and other participating assessment providers around Australia to ensure a successful trail and best possible outcomes for our older clients.
The IAT is a new assessment tool developed by the Department to better match services to a person’s aged care support needs. It will assess eligibility for all aged care programs, including home care, residential care, multipurpose services, and respite. IAT is specifically designed to make the assessment process more efficient, accurate and tailored to individual needs.
The tool will use a suite of assessment instruments that will cover several domains that identify a person’s aged care needs, including general and personal health, functional decline, cognition and behavior, psychological condition, community engagement and support, medical conditions, home and personal safety, and carer support.
We understand that not all senior Australians need intensive assessments, which is why the new tool will use trigger points to guide assessors to the most appropriate assessment level for each person. This approach ensures the assessment process is tailored to everyone’s needs.
The IAT tool has been developed through research and the tool will be further refined to ensure the best possible outcome for older Australians.
We are pleased to be part of this trail in aged care assessment and are looking forward to supporting older Australians through this new process.
