Lights, camera, action for Care Collective!

The Health Alliance’s Care Collective team have donned their directors’ caps, filming at Care Collective locations across Caboolture and Redcliffe in June.

The Care Collective project brings together 18 general practices and clinicians from Caboolture and Redcliffe Hospitals, working together with over 500 patients with Chronic Obstructive Pulmonary Disease, Congestive Heart Failure, and those with debility (frailty, risk of falls, dementia) to improve the coordination of their healthcare needs.

The Complex Care Coordinator (CCC) positions being trialled as part of the program provide an important connection role between hospital clinicians, community health providers and general practice to ensure patients are receiving coordinated, supported healthcare, close to home.

The short videos currently being filmed and set to premiere in July will highlight consumer success stories and detail the work the CCC’s are undertaking in their roles, working closely alongside the patient’s usual GP, community providers and hospital clinicians involved in the patient’s ongoing care.
