PHASES Webinar 3: General Practice Briefing Sessions - A Whole of Practice Approach to CVD Prevention

This session is part of a webinar series that will share the key details of the PHASES project and quality improvement activities being delivered across the state. PHASES is a statewide screening, recall and treatment initiative designed to improve the prevention and management of cardiovascular disease (CVD).

This session will explore:

• why CVD prevention remains critical in general practice
• how each member of the clinical team contributes to prevention workflows
• practical ways to embed CVD prevention without increasing consultation time
• key CVD risk parameters to improve risk identification and MBS eligibility
• at least three MBS items that support preventive CVD care.
