Striving towards our Strategic Goals: Brisbane North PHN’s 2024-25 Year in Review celebrates 12 months of impactful work commissioning to lay the foundation for the next era of service delivery across North Brisbane and Moreton Bay.

older people from priority populations were supported by care finders across the region.

GPs engaged with education events and activities delivered by our joint GP Liaison Officer (GPLO) program with Metro North Health.

episodes of care were delivered at Medicare Urgent Care Clinics (UCCs) across our region at Northside (Kedron), Murrumba Downs and Morayfield.

First Nations people received an extended or after-hours primary care service.

general practices participated in our Continuous Quality Improvement activities covering multicultural health, cancer screening, diabetes and vaccinations (COVID-19, flu and pneumovax).

young people received mental health support through headspace, with a total of 22,625 services delivered to meet their individual needs.

general practice teams supported 652 clients and delivered more than 3,120 interactions through general practice-based complex care coordination as part of the Care Collective.

people – 86 identified First Nations – received services via three Integrated Mental Health Hubs. 18,819 service contacts were delivered to meet individual support needs.

Our Strategic Goals, one year on
One year after releasing the Brisbane North PHN Strategic Plan 2024–2027, our strategic goals are embedded into the organisation to help create a community where good health is available to everyone. Faster and closer care for those with persistent pain
People living with persistent pain often face extended waits and travel long distances to access specialist care. The Specialist Pain Assistance Network (SPAN) set out to change that experience for people with persistent pain across the North Brisbane and Moreton Bay region.
In partnership with the Tess Cramond Pain and Research Centre at Metro North Health, the program brings together hospitals, primary care and allied health to provide faster care, closer to home. It has supported 120 patients over the past year, helping them access care an average of 489 days sooner and up to 600 kilometres closer.
