Brisbane North PHN’s Team Care Coordination team has been engaging with the community across the North Brisbane and Moreton Bay region, connecting seniors with practical support to help them manage their health, maintain their independence and continue living well at home.

Most recently, the team attended the Seniors Afternoon Tea in Scarborough hosted by Emma Comer MP, Member for Petrie, and joined the Health Alliance at the Seniors Morning Tea in Sandgate. These followed earlier community events in Bracken Ridge and the Longman Seniors Expos in Bribie Island and Caboolture.

These events give seniors, families and carers an opportunity to meet the team, ask questions and learn about health and community services available in their local area. They also help the PHN understand the issues that matter to the residents and strengthen connections with local organisations – a key part of being informed and led by the community.

Team Care Coordination is a free program that supports people aged over 18 who have complex and chronic health conditions. Clinical nurses and allied health professionals work directly with each person to understand their needs and goals, provide health education, and coordinate health, community and social supports.

Support can be provided through an in-home visit, telehealth appointment or phone consultation, with ongoing contact generally available for up to three months and extensions considered when needed. For seniors, this may include help to navigate services, build confidence in managing a health condition, reduce social isolation and connect with meaningful local activities.

People may be eligible if they have a chronic health condition affecting their life and live in the North Brisbane or Moreton Bay region. The program is not available to people living in residential aged care, and some people receiving comprehensive funding packages may require individual assessment. Individuals can refer themselves, while referrals can also come from GPs, practice nurses, hospitals and community providers.
