Commonwealth Home Support Program

Healthy@home is a consortium of 18 leading organisations who are experienced in supporting people with diverse needs, characteristics and life experiences.

Brisbane North PHN leads this consortium, which receives funding from the Australian Government Department of Health under the Commonwealth Home Support Program (CHSP).

CHSP supports people aged 65 years and over, or aged 50 years and over for Aboriginal and Torres Strait Islander people, who are still managing well at home, but could benefit from some extra assistance to continue living independently in the community.

Services are based on a wellness philosophy that supports the strengths, capacity and goals of older people to enhance independence, autonomy and physical, social and emotional wellbeing.

Healthy@home services include domestic assistance, personal care, individual and group social support, nursing, allied health, transport and specialised support services.
