The Mental Health Service Hubs deliver integrated clinical and non-clinical services for people with severe mental illness.

Hub services

  • assessment, triage and intake
  • service navigation
  • care coordination (including mental health nursing)
  • psychological therapy (groups)
  • physical health care
  • psychosocial support
  • assistance to access the National Disability Insurance Scheme (NDIS)
  • a range of in-reach services by external providers (for example alcohol and other drugs treatment, employment, housing and finance).

Eligibility criteria

  • 18 years and over
  • a diagnosable or severe and complex mental illness (episodic or persistent)
  • experiencing significant disruption to daily life, wellbeing and functioning
  • unable or unwilling to access NDIS
  • not accessing services through public hospitals
  • in need of clinical support that is not being provided by NDIS or hospital service.

Referral information

Anyone can refer to the Mental Health Service Hubs with the consent of the person being referred.

Hubs evaluation report

In 2021, an independent evaluation was completed regarding the Brisbane North mental health Hubs. The purpose of this evaluation was to explore whether this integrated approach improves outcomes for consumers.

The evaluation demonstrated that the Hubs were effective in engaging people living in the community with severe mental illness, led to improvements in psychological distress and recovery outcomes, and met the needs of service users.

Brisbane North PHN, hub staff, stakeholders and people with lived experience celebrated the results of the evaluation report at Communify in Bardon on Wednesday 24 August which was live streamed nationally.

The Head to Health phone service operates 8:30 am - 5:00 pm, Monday to Friday (excluding public holidays). Callers after hours will have the option to leave a message and receive a call back the next business day. Please note that Head to Health is not a crisis service.
