Telehealth care

Brisbane North PHN supports Residential Aged Care Homes across North Brisbane and Moreton Bay to use telehealth as part of routine resident care. Telehealth can improve access to healthcare professionals, support timely clinical decision-making, and help residents receive care in the comfort of their home.

Benefits of telehealth

When clinically appropriate, telehealth can help residents access care from GPs, specialists and other health professionals more quickly. It can support timely assessments, improve collaboration between healthcare providers, and reduce the need for residents to travel to appointments. Telehealth may also help minimise disruptions for residents and support care to be delivered in a familiar environment.

Suppporting telehealth in aged care

Brisbane North PHN continues to support Residential Aged Care Homes to strengthen telehealth-supported care through education, capability building, digital health initiatives and quality improvement activities.

This includes access to telehealth training resources, support for digital health capability, and initiatives that help aged care homes integrate telehealth into routine resident care.

Telehealth training program

To support the ongoing telehealth training needs of RACHs and general practice, Brisbane North PHN is one of 15 PHNs from across Australia to develop the Telehealth Training Program.

This microlearning series includes over 60 modules directed at RACHs and remote-end clinicians (and other health professionals) and is available for upload to your organisation’s Learning Management System. Access is free and can be requested by clicking on the button below.

You can also access the microlearning modules via the Aged Care Quality and Safety Commission’s Learning Information Solution (Alis) which is free for Australian government-funded aged care providers and their employees.
