Care Collective back Heart Week with coordinated approach to care
09 May 2024Heart Week, an initiative of the Heart Foundation, aligns with Australia’s national heart health awareness week and encourages the Australian public and their health professionals to start a conversations about heart health and heart disease prevention.
The Care Collective program operational in parts of our region supports the coordination of care across the system for people with complex chronic health conditions (including heart disease) by nurses who have been upskilled to work as dedicated Complex Care Coordinators (CCCs).
Since the program’s inception, Complex Care Coordinators working in primary care settings in the Caboolture and Redcliffe regions have supported 80 patients in the community with heart failure, with this number growing weekly.
CCCs fully understand the importance of heart health and the impact that improved education, communication and support can have on the health outcomes and quality of life of people experiencing heart disease.
Our primary Complex Care Coordination teams work alongside health professionals from the Caboolture and Redcliffe Hospitals on an as needs basis in a collaborative approach to addressing the holistic needs of a person with heart disease.
Heart Week is an opportunity to celebrate the impact that dedicated, coordinated care has had on the Care Collective’s heart disease clients, their families, and their ability to regain confidence in their ability to manage their own healthcare.
