Finding care for older Australians, together

Brisbane North PHN's care finders came together at our offices to network and share the progress of the care finder program.

The program is specifically designed to assist older Australians with complex needs who have no support or are isolated and who find it difficult or are hesitant to access aged care services.

At the heart of the program are care finders – dedicated individuals who provide intensive and practical face-to-face support to help these vulnerable people to access and navigate Australia’s aged care system.

Collectively, we now have over 20 care finders working across the North Brisbane and Moreton Bay region.

Brisbane North PHN’s six commissioned care finder service provider organisations include Communify Queensland, Footprints Community, Micah Projects (Lotus Place), OzCare, Queensland Council for LGBTI Health and World Wellness Group.
