Principles of Chronic Conditions Management program: Bringing it all together - developing a care plan
Hosted by APNA
Hosted by Metro North Health and Brisbane North PHN
Metro North Health and Brisbane North PHN invite General Practitioners (GPs) to join our experts for an evening of education and discussion hosted by the GP Liaison Officer team.
Join your colleagues and two local rheumatologists for a practical education evening. Refresh your knowledge on spotting rheumatology conditions early, then consider cases where GPs have used Request For Advice (RFA) to resolve rheumatology dilemmas from their desktop.
There will be opportunities to engage with our experts for advice on management, referral pathways and support services that may help you and your patients. Feel free to bring your own Requests For Advice along too!
This interactive session will be held in person only.

We acknowledge the Traditional Custodians within our region: the Jagera, Turrbal, Gubbi Gubbi, Waka Waka and the Ningy Ningy peoples of where we meet, work and learn. Brisbane North PHN is committed to reconciliation. Our vision for reconciliation is where the stories of our First Nations’ people are heard and shared, and networks are formed.
