Brisbane North PHN is hosting an evening workshop on multidisciplinary team (MDT) care to explore the importance of, and ways to improve, MDT working in residential aged care homes (RACHs). This initiative has come from feedback recognising that MDT working is more complex in the RACH setting, where members of the team are coming together from various organisations to collaborate on providing the best possible care for residents. This is a unique opportunity to come together and explore ways to improve MDT working in your facility and how this can be supported through the General Practice in Aged Care Incentive (GPACI) and the Aged Care On-site Pharmacy measure (ACOP).

These workshops are free, with a two-course dinner and drinks included. There will be free parking on site.
To get the most out of this event, we strongly recommend coordinating with a few members of your MDT to attend as a group, for example, a representative from the RACH, a GP that visits the facility, and an aged care onsite pharmacist onsite or allied health clinician who supports resident care. This will allow for conversations and planning in the interactive part of the evening to support practical outcomes.
