The General Practice in Aged Care Incentive (GPACI) aims to encourage GPs and members of their teams to provide more proactive, planned and continuous care planning to improve the quality of general practice care for residents in residential aged care homes.

To support GPs participation in GPACI, Brisbane North PHN has developed a portal for GPs to submit an expression of interest (EOI) form to indicate their availability and interest in providing their services to aged care facility residents.

The information provided via this EOI will be published on our website to assist RACHs identify and directly contact GPs who match the needs of their residents who are participating in this measure.

Once a matching service provider or facility has been identified, please remove your EOI via the portal (link here). Unmatched EOIs will expire automatically after three months.
