Brisbane North PHN in partnership with Metro North Health, are currently conducting a health needs assessment to identify the health and service needs of the region.
The PHN would like to consult with residents and health practitioners of the North Brisbane and Moreton Bay region about the challenges that impact them or their patients in achieving good health and wellbeing.
The PHN invites residents and health practitioners to participate in a survey that will build the PHN’s understanding of:
Responses to the survey will help the PHN to better understand the community and will be examined alongside data about our local population, the availability and location of services and how they are used.
Following the collection of this information, a document will be prepared that summarises the health needs and service gaps in the North Brisbane and Moreton Bay region. The purpose of the health needs assessment is to:
The development of the health needs assessment as a resource will assist the PHN in subsequently working with community and health services to develop and deliver evidence-based plans to address health needs and service gaps.
In addition to engaging with community and practitioners, our health needs assessments are developed in consultation with key stakeholders, including our local hospital and health services, clinicians, service providers and community and consumer representatives. In working with Metro North Health, we work towards having a shared understanding of the community’s health and service needs and a collaborative effort in addressing these needs together.
The PHN is dedicated to listening to our community, and endeavour to build the capacity of people and communities to influence and lead system design. All 31 PHNs across the country are currently undertaking a health needs assessment.
