Our Year in Review: Brisbane North PHN in 2020/21

We invite you to take a look at our Year in Review, to see what we accomplished in 2020/21 and the outcomes we achieved together with our community partners.

This year, the PHN delivered practice support, commissioned a range of community services and welcomed many new areas of work. We did this while responding to the COVID-19 pandemic – making sure we supported general practices and other healthcare providers to keep our community safe.

Some of the highlights this year include:

  • supporting four GP-led respiratory clinics and 213 vaccination clinics with COVID-19 testing and vaccination
  • supporting 300+ clients following a suicide attempt or suicidal crisis
  • welcoming our first Indigenous Australian, Jody Currie, to the Brisbane North PHN Board of Directors
  • working with 10 community aged care providers and training 47 support workers to deliver the Active at Home program
  • My Mental Health Service Navigators responding to 1,725 enquiries: providing information, advice or referring into other services
  • providing 7,713 older people with services from the Commonwealth Home Support Program
  • supporting 1,316 people who have complex chronic health conditions through our Team Care Coordination program

We thank our community partners for their ongoing support, and the energy they bring to our many and varied programs of work across the North Brisbane and Moreton Bay region.
