Late last year Brisbane North PHN was proud to launch an information booklet designed to assist patients accessing the National Disability Insurance Scheme (NDIS).
Patient Information Booklet: Accessing the National Disability Insurance Scheme was launched at Changing Lives, Changing Communities in Caboolture, part of the Queensland Council of Social Services statewide series of events.
The booklet, funded by Partners in Recovery and developed by Queenslanders with Disability Network, was produced in conjunction with a resource for GPs, General Practice Toolkit: Understanding your role in the NDIS.
The booklet provides patients with information to prepare them for a GP visit to discuss the NDIS and complete their access request form.
