We’ve just published our Year in Review, showcasing Brisbane North PHN’s highlights and accomplishments from the 2021/22 financial year. The need for primary care services continues to rise – with more demand for mental health, aged care and chronic health support.

In response, we have: 

  • delivered and commissioned 76 programs to improve the health and wellbeing of North Brisbane and Moreton Bay residents 
  • supported 1,292 clients with complex or chronic health conditions, totalling 14,220 client interactions 
  • developed 663 localised HealthPathways, with the webpage viewed 145,300 times 
  • made 1,373 commissioned visits to residential aged care facilities impacted by COVID-19 outbreaks  
  • delivered 205,360 aged care service sessions to support 8,995 older Australians 
  • supported 342 general practices 

This is just a snapshot of the year. Inside, you’ll find stories about how our programs and commissioned services have impacted real people in our community.
