Brisbane North PHN supported the Moreton Bay Dementia Alliance Walk with Me community event, held on 23 September during Dementia Action Week.

Up to 472,000 Australians are living with dementia. Dementia Action Week is a week dedicated to raising awareness, reducing the stigma, and showing our community support for people living with dementia.

This year's focus was supporting and celebrating those who care for people living with dementia.

The Walk with Me community event was a morning filled with presentations, education stalls, a complementary morning tea and a walk around the beautiful Lake Eden at North Lakes.

The event also celebrated the recent launch of Your Care Navigator, the new online tool for dementia support and information developed by Brisbane North PHN.

The Your Care Navigator website is now live and is the ultimate guide for finding support, services and information relating to dementia in the North Brisbane and Moreton Bay area.

The easy-to-use online tool will point you in the right direction for:

  • emergency respite
  • help at home
  • mental health and wellbeing
  • planning for the future
  • education and resources.

The PHN collaborated with peak national body Dementia Australia and creative agency New Word Order to create this tool.
