Aged Care Forum
Our Aged Care Forum is held twice a year and includes a program of speakers who are experts in their fields and bring valuable information and perspectives to support the aged care sector. Attendance at the forum also offers an opportunity to network and build connections with a broad range of health and aged care professionals across the Brisbane North and Moreton Bay regions.
RAC Collaboratives
Brisbane North PHN hosts two Residential Aged Care (RAC) Collaboratives each year where we welcome health professionals and professional caregivers working in RACHs, hospital and primary care in the Brisbane North region to join in our shared goal to improve the care journey for residents of RACHs through information sharing, feedback and codesign of targeted solutions.
Brisbane North Palliative Care Collaborative
The Brisbane North Palliative Care Collaborative aims to improve the palliative care journey for people in the North Brisbane and Moreton Bay region by facilitating stakeholder representative networking, information exchange and shared learning.
