Residential aged care homes seeking ACOPs
Brisbane North PHN has developed a portal for RACHs to submit an expression of interest (EOI) form to seek GPs for their residents.
The following ACOPs/Pharmacies have expressed an interest in providing support to aged care home residents.
To enhance the security of information provided during the EOI process, you will be prompted to complete a reCAPTCHA sign in box before you can view the list of EOI submissions.
Once the reCAPTCHA has been completed, you can view the full details of all active expressions of interest (EOI) by selecting the View button beside each entry.
Connecting with a matching ACOP
If you are interested in seeking the services of a ACOP listed below, please contact them directly via the 'Contact Request Form' button at the bottom on their EOI.
You will be asked to provide your contact name, email address, phone number (optional) and any comments regarding your request.
Submitting an EOI
If you cannot find an available ACOP to suit the needs of your residents, you can submit your own EOI via the Care Provider Connect portal.
Logging in to your account
Brisbane North PHN have created a user account on behalf of all RACHs in the Brisbane North region. These details have been provided to your facility via email.
