Residential Aged Care Collaboratives
Collaborative groups bring individuals and organisations together to identify local issues, problem solve and promote communication that supports coordination of care for people in the community.
Brisbane North PHN hosts two Residential Aged Care (RAC) Collaboratives. Our goal is shared across RACHs in our region and is to improve the care journey for residents of RACHs through information sharing, feedback and codesign of targeted solutions.
Our RAC Collaboratives welcome health professionals and professional caregivers working in RACHs, hospital and primary care in the Brisbane North region:
- RAC Collaborative (North) – welcomes health professionals and professional caregivers working in RACHs, hospital and primary care in the Moreton Bay Council and Sommerset Regional Council areas of the PHN region
- RAC Collaborative (South) - welcomes health professionals and professional caregivers working in RACHs, hospital and primary care in the Brisbane City Council areas of the PHN region (north of the Brisbane River).
The RAC Collaboratives meet in person, twice yearly (usually May and October) for half a day. Meetings are free to attend and include refreshments. Typically, our RAC Collaborative meetings include:
- updates from Metro North Health and a GP representative
- a world café session for participants to provide feedback on key topics, and assist in the codesign of shared resources / improvement actions
- opportunities for networking with colleagues and RAC support services.
