General practitioners play a key role in shaping the health of our community and helping patients understand the importance of making lifestyle changes for better health – now and in the future.
Brisbane North PHN is proud to support the My health for life program – a free program to improve the health and wellness of Queenslanders.
The program is designed to support people to make healthy and sustainable lifestyle changes, particularly those at high risk of developing type 2 diabetes, stroke and heart disease.
My health for life offers eligible participants aged 18+ years a personalised and structured six-month lifestyle modification program. Individuals receive support to help them change habits so they can reach their personal health goals.
A team of health professionals – including dietitians, nurses, psychologists and exercise physiologists - all work together to help participants understand:
The My health for life program is funded by Health and Wellbeing Queensland and managed by the Healthier Queensland Alliance, of which Brisbane North PHN is a member.
