Making sense of Primary Sense: webinar explores data-driven clinical support tool

Brisbane North PHN provides Primary Sense for free to practices in our region as part of our commitment to support general practice. Primary Sense is a population health management, clinical decision support and data extraction tool that uses de-identified data and evidence-based algorithms to enhance a GP’s ability to manage patient care in the community.

What can Primary Sense do for my practice, my patients and our population?

Primary Sense was originally developed with and for local GPs to maximise the value immediately returned to GPs from their data through the clinical prompts, alerts and reports that are delivered at the point of care.

The meaningful and real-time information that Primary Sense provides supports GPs to:

  • run clinical audits using the in-practice desktop auditing tool
  • identify high risk groups and link appropriate interventions (e.g. vaccinations)
  • identify patients at risk of poor health outcomes or chronic disease
  • reduce potential adverse reactions to medication, through medication safety alerts
  • avoid preventable hospitalisations and provide the right care at the right time.

Primary Sense is powered by the world’s leading population health analysis tool, the John Hopkins University ACG® System. For practice managers, the software (compatible with major practice management platforms) can be quickly installed, easily onboarded, used by both clinical and non-clinical staff with little to no training, and makes it easier to meet PIP QI documentation requirements by delivering on demand one-click reports.

By improving the capacity of general practice to provide responsive clinical patient support at an individual level, Primary Sense can also help the PHN to positively shift health outcomes at a population level.

Primary Sense data allows the PHN to accurately identify community risk of inequitable or adverse health outcomes, surmise specific health needs in our region, inform quality improvement activities for primary care, and appropriately target service funding, programs and advice in a timely manner.

Next month, GPs and practice staff are invited to join our Quality Improvement and Development team and a panel of GPs from our region online to learn how Primary Sense can be incorporated into practice, including:

  • using Primary Sense to assist in meeting PIP QI measures, Continuous Quality Improvement (CQI) and accreditation
  • how prompts, alerts and reports can support GPs at the point of care.
