Brisbane North PHN’s 2024–25 Year in Review celebrates 12 months of delivering impactful primary care services across the North Brisbane and Moreton Bay region – laying the foundation for even more efficient, accessible and affordable healthcare.
This year, we’ve had more impact than ever before, working together to build a fairer and more inclusive healthcare system so residents can access care closer to home.
The numbers tell just part of the story, with 209,500 PHN-commissioned services delivered to more than 88,500 people in our region.
Three Medicare Urgent Care Clinics delivered 62,264 episodes of care when it was urgent but not an emergency – underscoring the vital contribution of this service after just 18 months of operation.
And, of the 1,910 clients supported by Team Care Coordination, 91.3 per cent were better able to cope with their chronic health condition.
But it’s the people in our community who illustrate the real impact of having accessible, affordable primary care.
These are just some of the outcomes of working with communities and healthcare providers to deliver primary care that’s relevant, close to home, and inclusive.
