RACH expression of interest
Brisbane North PHN is working with general practices and Residential Aged Care Homes (RACHs) to support better access to GPs for RACH residents.
Our work has been enabled by the introduction of the General Practice in Aged Care Incentive (GPACI) by the Australian Government Department of Health, Disability and Ageing (DHDA) in July 2024. This is the first incentive of the MyMedicare program.
GPACI aims to improve the quality of care for residents living in RACHs by strengthening and formalising relationships between residents and their GP or general practice. GPACI promotes regular visits from their GP and additional care planning services to ensure continuity of care and prevent avoidable hospital admissions.
General practices and GPs will receive incentive payments for visiting residents of RACHs. Residents must register with MyMedicare in order for their GP and general practice to receive the incentive payments.
RACHs can help GPs by:
Brisbane North PHN has consulted RACHs, GPs, consumers and other key stakeholders to understand the current state of GP access for residents of RACHs in our region. A summary of what we heard can be found in the RACH Resident Access to Primary Care Report December 2024.
Brisbane North PHN is working with an expert advisory group to develop resources and interventions that will support better access to GPs for residents of RACHs. We will continue to update these resources as they become available.
Brisbane North PHN have developed a portal to provide a central avenue for GPs and RACHs to express their requirements and availability, enabling them to partner up to provide greater care to aged care home residents.
To enable better access to GPs for RACH residents within our region, Brisbane North PHN has created a portal for:
The information provided through this EOI process will be published on our website to assist relevant parties to identify opportunities where their requirements, services and availability are a suitable match for each other.
To submit an EOI via the portal, a user account needs to be created.
A user account has been created on behalf of all RACHs in the Brisbane North region, and these details have been provided to your facility via email.
If you have forgotten your account details or are having difficulty accessing your account, please contact us for assistance by selecting 'x' as the enquiry type from the menu options available on the Contact Form, and a member of our team will get in touch with you.
We acknowledge the Traditional Custodians within our region: the Jagera, Turrbal, Gubbi Gubbi, Waka Waka and the Ningy Ningy peoples of where we meet, work and learn. Brisbane North PHN is committed to reconciliation. Our vision for reconciliation is where the stories of our First Nations’ people are heard and shared, and networks are formed.
