GP Access

Brisbane North PHN is supporting better access to GPs for Residential Aged Care Home (RACH) residents by facilitating stronger relationships between general practices and RACHs.

General Practice in Aged Care Incentive (GPACI)

Our work has been enabled by the introduction of the General Practice in Aged Care Incentive (GPACI) by the Australian Government Department of Health, Disability and Ageing (DoHDA) in July 2024. This is the first incentive of the MyMedicare program.

GPACI aims to improve quality of care for residents living in RACHs by strengthening and formalising relationships between residents and their GP or general practice. GPACI promotes regular visits from their GP and additional care planning services to ensure continuity of care and prevent avoidable hospital admissions.

General practices and GPs will receive incentive payments for visiting residents of RACHs. Residents will need to register with MyMedicare in order for their GP and general practice to receive the incentive payments.

RACHs can help GPs by:

  1. ensuring they know about GPACI and are ready to answer questions from residents and their families. The Department has developed an Information Kit for Residential Aged Care Homes to help with this activity
  2. providing information to residents and their families about the new incentive and how they can register for MyMedicare. The Department has developed an Information Kit for Aged Care Residents, their Families and Carers for this activity.
  3. speaking to GPs about how they can assist with the process of registering residents. For more information about how residents can register, refer to the MyMedicare information page of the Department of Health, Disability and Ageing website.

Facilitating better access to primary care

Brisbane North PHN has consulted RACHs, GPs, consumers and other key stakeholders to understand the current state of GP access for residents of RACHs in our region. A summary of what we heard can be found in the RACH Resident Access to Primary Care Report December 2024.

Brisbane North PHN is working with an expert advisory group to develop resources and interventions that will support better access to GPs for residents of RACHs. We will continue to update these resources as they become available.

Expression of interest for participating GPs and RACHs

To support the implementation of GPACI within our region, Brisbane North PHN has created a portal for:

  • GPs and general practices to advertise their availability to provide services to aged care residents
  • RACHs to advertise they require GP assistance.

RACHs, general practices and GPs can submit an expression of interest (EOI) form via the portal.

The information provided through this EOI process will be published on our website to assist relevant parties to identify opportunities where their requirements, services and availability are a suitable match for each other.

How to submit an EOI

For first time users, simply register a new account to create a user profile.

If you do not receive an email notification following registration, please check your email inbox spam folder, or contact us for assistance activating your account.

Once registered, you can simply log into your account by selecting the relevant option below to post a new submission.

Identifying a matching service provider or facility

GPs, practices or RACHs who identify a service provider or facility that matches their requirements, should contact the other party directly via the contact details provided on the EOI.

Once a matching service provider or facility has been identified, please remove your EOI via the portal (link here). Unmatched EOIs will expire automatically after three months.

Please contact (link to Contact Us form) the Aged Care projects team should you require further assistance or have any questions regarding the EOI process.
