Care Provider Connect

Brisbane North PHN have developed a Care Provider Connect portal to provide a central avenue for GPs and Residential Aged Care Homes to express their requirements and availability, enabling them to partner up to provide greater care to aged care home residents.

Interested GPs and RACHs can use the Care Provider Connect portal to:

  • submit an expression of interest (EOI) form
  • search for a service provider or facility matching their needs and requirements and contact them directly via a secure messaging service
  • subscribe to receive notifications when a new EOI submission matches their requirements.

Using the Care Provider Connect portal

You do not need to have a user account to view the list of currently available EOIs, however a user account is required to submit an EOI via the portal.

General practitioners

For first time general practice users, simply register to create an account.

If you do not receive an email notification following registration, please check your email inbox spam folder, or contact us for assistance activating your account.

RACHs

An account has been created by Brisbane North PHN on behalf of all RACHs in the Brisbane North region. These details have been provided to your facility via email.

If you have forgotten your account details or are having difficulty accessing your account, please contact us for assistance, and a member of our team will get in touch with you.

The information you provide via the EOI form will be published on our website and includes:

  • your name and practice name (as relevant)
  • your preferred region/s to work in
  • the number of residents you have capacity to see
  • the model of service delivery you can provide and details of any special interest areas.

You will be asked to provide your contact email address, however these details are only provided for the purposes of the PHN notifying you via email of a successful contact request.

Your email address will not be published on the public facing website.

The information you provide via the EOI form will be published on our website and includes:

  • your RACH name and address
  • region you are located within
  • total number of beds at your RACH and the number of residents seeking GP services
  • profile of your resident cohort
  • the RACH provided supports available to a visiting GP
  • details of any restrictive practices at your facility.

You will be asked to provide your contact email address, however these details are only provided for the purposes of the PHN notifying you via email of a successful contact request.

Your email address will not be published on the public facing website.

GPs, practices or RACHs who identify a service provider or facility that matches their requirements, should contact the other party directly via the Contact Request form button located at the bottom of the individual EOI listing.

An email notification will be sent via the PHN website to the owner of the EOI providing the contact details of the interested party.

The GP or RACH receiving the EOI enquiry can contact the other party directly on the contact details provided.

Should you require further assistance or have any questions regarding the EOI process please contact the Healthy Ageing team via the Contact Form by selecting 'Care Provider Connect portal' as your enquiry type.

You can subscribe to receive an email notification when a new EOI is submitted that matches your EOI against all of the following fields:

  • region RACH located within
  • number of residents seeking GP services
  • RACH provided supports available/minimum requirements of GP

To subscribe for email notifications, you need to have a user account.

Once your account has been created, select the 'Subscribe for matching notifications' checkbox located at the top of the User dashboard.

Once a matching service provider or facility has been identified, please log in to your user account and deactivate your EOI. To assist us in determining the effectiveness of this measure, you will be asked to provide a reason for deactivating your EOI submission.

All EOIs will automatically expire three months post their submission date.

You can log in to the User dashboard and easily edit your EOI, using the 'Edit' button on your active EOI.

If your EOI has expired, you will need to submit a new EOI via the User dashboard.
