The General Practice in Aged Care Incentive (GPACI) supports older people living in Residential Aged Care Homes (RACHs), supporting them to receive quality primary care services within their aged care homes.

To better support aged care residents to participate in this incentive, Brisbane North PHN has developed a portal for RACHs to submit an expression of interest (EOI) form to outline the primary care needs of their residents, making it easier for GPs to identify RACHs requiring their services.
The information provided via this EOI will be published on our website to assist GPs identify and directly contact, RACHs requiring GP care for their residents under this measure.

Once a matching service provider or facility has been identified, please remove your EOI via the portal (link here). Unmatched EOIs will expire automatically after three months.
