Care Provider Connect extended to ACOPs and pharmacy providers

Based on feedback from the community, as well as the PHN’s expert advisory committee, Brisbane North PHN’s Care Provider Connect portal has now been extended to include matching opportunities for Aged Care On-site Pharmacies (ACOPs) and pharmacy providers.

Launched in September 2025, the portal was originally developed to address the need identified for a centralised platform to help GPs and Residential Aged Care Homes (RACHs) share their requirements and availabilities and rapidly connect with one another.

Now, ACOPs or pharmacy providers can also express their interest in working in RACHs and RACHs can advertise their need for an ACOP via Care Provider Connect.

All parties can use the portal to search for a service provider or facility matching their needs, submit an expression of interest (EOI) and subscribe to receive notifications when a new EOI matches their requirements (if one is not already available).

The Healthy Ageing team at Brisbane North PHN said they are thrilled the program has been extended to include ACOPs and pharmacy providers as it will facilitate greater care for aged care home residents.

'We are so pleased we have been able to facilitate the extension of Care Provider Connect to include ACOPs,’ Brisbane North PHN Healthy Ageing - Lead Sahba Dehghani said.

‘This is a big win for the sector, with more and more RACHs looking to employ an on-site pharmacist to ensure the highest quality of care for residents and improve medicine safety in the home.

‘Now both RACHs and ACOPs have a central place to find a good match and are able to reach out directly to avoid delays.’
