Chronic Disease Care Planning

Your practice is invited to join a full day virtual training course Chronic Disease Care Planning to enhance your team’s skills in managing chronic disease care.
Clinical primary care staff, including GPs, practice nurses, registered nurses and enrolled nurses are encouraged to attend and learn how to:
- develop patient care plans for chronic diseases
- implement health interventions that identify risk factors
- support behaviour changes that lead to improved health outcomes.
Pre-requisite:
Students must complete the Foundations of Chronic Disease online module to enrol in this program.
This is a fully funded online offering with limited places available.
