Help to navigate and refer into mental health, suicide prevention and alcohol and other drug services in the North Brisbane and Moreton Bay region.


Medicare Mental Health phone service
Medicare Mental Health can connect people with mental health services in the Brisbane North and Moreton Bay region and is the preferred gateway for accessing Brisbane North PHN‑commissioned mental health services.
Medicare Mental Health phone line 1800 595 212 can be accessed by consumers, their families, carers, as well as GPs, service providers and other health professionals.
The Medicare Mental Health phone line will operate 8:30 am - 5:00 pm, Monday to Friday (excluding public holidays). Services incur no out-of-pocket cost.
To make a referral to Medicare Mental Health for service navigation support, you can:
- Call 1800 595 212
- Submit a secure eReferral using the HealthLink SmartForm, embedded in GP practice software, or via the Health Professional portal (instructions below)

HealthLink SmartForm eReferral
The HealthLink SmartForm eReferral provides a simple, secure and trackable way to submit referrals directly to the Medicare Mental Health phone service for Initial Assessment and Referral (IAR).
Embedded within GP practice software, HealthLink SmartForms allow referrals to be completed and sent electronically, reducing paperwork and administrative follow‑up. Once submitted, referrals are received directly by the Medicare Mental Health phone service, supporting timely assessment and connection to the most appropriate mental health care.

Based on the IAR Level of Care, demographics, consumer preference and eligibility, Medicare Mental Health phone service may direct those referred to PHN‑commissioned services or other suitable providers. Referrers will be notified of outcomes accordingly.

All PHN mental health and suicide prevention services funded by the Department of Health are required to collect the Primary Mental Health Care Minimum Data Set (PMHC-MDS). This dataset improves the quality of services and allows the Department of Health to plan into the future. The PHN will collect this data on behalf of the Department of Health, however your consent to share this data with the Department of Health is optional. Information about the consent process is listed below.

Medicare Mental Health Centres
Medicare Mental Health Centre Services are now available in the Brisbane North and Moreton Bay region.
Medicare Mental Health Centres provide immediate, short to medium-term, mental health supports and service navigation. Centres welcome adults experiencing emotional distress, crises, mental ill health, and/or addiction, and their families and carers through a ‘no wrong door’ approach. Each Centre is staffed by qualified mental health professionals, working alongside people with lived experience, who will listen to your needs and work with you to ensure you receive the right supports, at the right time.
Medicare Mental Health Centres are here for everyone-even if it’s your first time seeking support. Services incur no out-of-pocket cost, and you don’t need an appointment or a GP referral - you can simply walk in.

Federal Assistant Minister for Mental Health and Suicide Prevention, Emma McBride MP, and Queensland Minister for Health, Tim Nicholls MP, officially opened Medicare Mental Health Centre sites at Inner-North Brisbane (at Lutwyche), Strathpine and Caboolture.

GPs and other health professionals can access an expanded range of Brisbane North PHN commissioned mental health, alcohol and other drugs and suicide prevention services via the PHN Commissioned Services and Medicare Mental Health eReferral.
