Metro North Health Forum 2025
Brisbane North PHN and Metro North Health jointly host the 2025 Metro North Health Forum
Aged and community care
Our aim is to improve health outcomes for people in our community with a focus on people 65 and over. To achieve this, we collaborate with consumers, service providers, hospitals, GPs and government.
Support for palliative care
We work with a range of stakeholders to improve the palliative care journey for residents of the North Brisbane and Moreton Bay region.
Regional assessment service
The Regional Assessment Service is no longer operating.
Collaboratives - Palliative Care and Residential aged care
Collaborative groups bring individuals and organisations together to identify local issues, problem solve and promote communication that supports coordination of care for people in the community.
Brisbane North Allied Health Collaborative
Allied health professionals from local private practice, non-government organisations and the Metro North Hospital and Health Service are working with Brisbane North PHN to implement a local collaborative model.
Mental health, alcohol and other drug and suicide prevention services
Brisbane North PHN works with local communities, consumers, carers, health professionals, hospitals and community mental health providers to design local solutions to meet local needs, via our joint regional plan 'Planning for Wellbeing'.
Brief therapy psychological services
Brief therapy psychological services are an evidence-based first option for people who are experiencing mild depression and anxiety.
Brisbane MIND and Brisbane MIND4KiDS
Brisbane North PHN’s Brisbane MIND program improves access to quality primary mental healthcare for vulnerable and disadvantaged communities across our region.
Services for people with severe mental illness
The Mental Health Service Hubs deliver integrated clinical and non-clinical services for people with severe mental illness.
Suicide prevention services
We fund a number of programs to deliver suicide prevention services in our region and we are committed to fostering collaborative partnerships with consumers and carers at each stage of the commissioning process.
Alcohol and other drug treatment services
Alcohol and other drug support and treatment services are available for people with concerns about their substance use.
Youth mental health services
Brisbane North PHN funds youth mental health services that provide psychological intervention to children, young adults and their families.
Aboriginal and Torres Strait Islander health and wellbeing
We're committed to improving the health outcomes of Aboriginal and Torres Strait Islander people in the North Brisbane and Moreton Bay region.

July marked National Diabetes Week, which aims to improve ongoing clinical outcomes and drive meaningful conversation around the impact of diabetes in Australia.

Your Care Navigator is a website designed to direct older people, their families and their carers living in the North Brisbane and Moreton Bay region to services and organisations that can support their needs in healthy living and ageing.

Innovating Residential Aged Care Homes in our region with Telehealth
In the last 12 months, 72 (representing 85 per cent) of Residential Aged Care Homes (RACH) in the north Brisbane and Moreton Bay region have received a telehealth grant to upgrade their internet connectivity and digital health equipment.

Partners 4 Health Regional Assessment Service took part in the Department of Health’s new Integrated Assessment Tool (IAT) Live Trail, which ran from April to late July this year.

Early intervention project supports people to manage chronic conditions
In July 2021 the Department of Health and Aged Care (DOHAC) provided funding to the Brisbane North PHN to commission services that focus on early intervention initiatives that support healthy ageing and ongoing management of chronic conditions.
Local insights inform GP workforce need and training capacity in Queensland
From May to July this year, local PHN GP Workforce Planning and Prioritisation (GP WPP) program teams conducted over 900 consultations across the state to capture key data and workforce insights from local primary care stakeholders.

Supporting chronic disease management this World Diabetes Day
World Diabetes Day was established by the International Diabetes Federation and the World Health Organisation (WHO). This worldwide campaign, held on or around November 14 each year, is an important awareness raising event, ensuring diabetes remains top of mind publicly and politically. The “special” date marks Sir Frederick Banting’s birthday who – in partnership with Charles Best – discovered insulin in 1922.

My strength lies in uncovering what's at the heart of the opportunity, and engaging deep within organisations, communities and customers, bringing them together to navigate the complexity for meaningful, impactful, change. My work spans sectors such as technology, healthcare, policy, not-for-profi t, fi nancial services, and education, grounded in a deep understanding of psychology, design, and technology.
